Application Process

Thinking about becoming a Laker? At Great Lakes, we’re committed to providing a high quality Christian education, and we want to make the application process as smooth as possible for you.  Our application packaged includes several sections, so be sure to follow the checklist to guide you through each step. We look forward to receiving your application and getting to know you better!

ALL OF THE FOLLOWING DOCUMENTS ARE REQUIRED UNLESS OTHERWISE SPECIFIED:

• Online Application Form
• Copies of Report Cards for two previous years and I.E.P. if applicable – Send Application Attachments
• Statement of Community Expectations – Sign Community Expectations and Standards Statement
• Student Technology Use Agreement – Sign Technology Use Agreement
• Medical Form –Complete Medical Form Online
• School Immunization Records – Send Application Attachments
• Permissions – Complete Permission Form Online
• Letter of Consent to Transfer OSR – Complete OSR Form Online
• Copy of Government Issued ID: Birth Certificate (submit a photo) or Passport – Send Application Attachments
• Custodian Information Required for International Students Only –Complete Custodian Form Online
